Gazette Name Change Notification Format and Sample: What the Notice Says, on Both West Bengal Routes
Four documents carry the same facts in a gazette name change: the affidavit, the newspaper notice, the gazette form, and the published notification. If any one of them disagrees with the others, the file is returned. This page shows the wording of each, with samples, for the Kolkata Gazette Part II and the Gazette of India.

What Is the Format of a Gazette Name Change Notification? (Quick Answer)
A first-person declaration with five parts: who (your existing name, son/daughter/wife of guardian, residential address), what (changed my name from old to new), basis (affidavit number and where it was sworn), effect (henceforth I shall be known as the new name and my documents should be corrected), and identity (both names are one and the same person). The Kolkata Gazette Part II uses this exact declaration on its portal. The Gazette of India prints the substance of your signed proforma as a numbered entry in Part IV. The affidavit and the newspaper notice carry the same facts, and all four must agree word for word.

Gazette Notice Wording Rules in West Bengal: The Fixed Points
- The five fields
- Old name, new name, parentage or husband's name, full address, and a declaration that both names refer to the same person. Every notice on either route carries these.
- Word the same everywhere
- The affidavit, both newspaper notices and the gazette form must carry the same names, the same spellings and the same address. A single letter of difference is the commonest reason a file comes back.
- Signature
- Signed in the old name on every document. The new name is not legal until the notification is published.
- State declaration
- Kolkata Gazette Part II, entered online at wbgazettepart2.in with the annexure undertaking: Annexure I for a name, II for a surname, III for religion, IV for gender.
- Central notice
- Gazette of India Part IV, typed on the prescribed proforma from deptpub.gov.in, signed in the old name, with two witnesses giving names and addresses.
- Affidavit attestation
- State route: a First Class Judicial Magistrate. Central route: a Notary Public. The wording is otherwise the same.
- Newspaper wording
- The same text in one Bengali daily and one English daily. Book classified text, not display, and read the proof before it prints.
- Newspaper proof
- Complete original printed pages with the masthead and date visible. A trimmed cutting is refused on both routes.
- Languages
- Kolkata Gazette Part II accepts Bengali, English, Hindi, Urdu, Nepali and Santali. The Gazette of India file is read in English.
- Address must match Aadhaar
- UIDAI expects the address in the notification to match your Aadhaar address, so check Aadhaar before drafting, not after publication.
- Error after publication
- Central route: a corrigendum, applied for within six months of publication. State route: redressal within a week, subject to submission of the registered document.
- Free samples
- Real published notices are free to read: the Monthly Gazette tab on wbgazettepart2.in and the Part IV issues on egazette.gov.in. Nobody should charge you for a format.

Matching the Affidavit, Newspaper Notice and Gazette Form: Four Fields
Before you pay any fee, lay the affidavit, the newspaper proof, the gazette form or portal declaration, and your ID side by side. Read these four fields across all of them:
- Existing name: spelling, spacing, initials, exactly as on the ID you will upload or attach
- New name: the final form, decided once; Aadhaar allows only two name changes in a lifetime
- Guardian's name: father's, or husband's; for a married woman, both, and the same spelling everywhere
- Residential address: matching the ID and, for Part II, the domicile proof
Why this page exists
Most returned files are not about eligibility or fees. They are a middle initial present on one document and absent on another, or a flat number that moved between the affidavit and the notice. The gazette office does not correct it for you; it sends the file back and the fee stays paid.

Common Questions People Search About Gazette Notification Format
Three short answers to the searches that bring people to this page.
Is there an official gazette name change format PDF to download?
For the central route, yes: the prescribed proforma is downloaded free from deptpub.gov.in and is the only form the Department of Publication accepts. For the state route there is no separate form to download, because the declaration is typed into the wbgazettepart2.in screens and the undertaking is the annexure the portal shows you. Any site selling you a format is selling a free public document.
Can I write the gazette notice in Bengali?
On the Kolkata Gazette Part II, yes. The guideline accepts Bengali, English, Hindi, Urdu, Nepali and Santali, so a Bengali notice is normal and needs no translation. For the Gazette of India, keep the notice and the file in English, because that is how the Delhi office reads it. If you publish one Bengali and one English newspaper notice, you are covered either way.
What happens if the newspaper prints my name wrongly?
Stop before you file. A misprint in the notice breaks the match between the newspaper, the affidavit and the gazette form, and the file will come back. Ask the paper for a corrected republication, keep both pages, and file with the correct one. Reading the proof before it prints costs nothing and saves a second fee.

Gazette Notice Format Terms Explained: Deponent, Proforma, Corrigendum
Eight words that appear in the formats on this page, in one line each.
Deponent
The person swearing the affidavit, meaning you. The signature block at the foot of an affidavit is the deponent line, signed in the old name.
Verification clause
The closing line of an affidavit stating that everything above is true to your knowledge, with the place and date. An affidavit without it is incomplete.
Proforma
The prescribed Gazette of India application form from deptpub.gov.in. It must be typed, never handwritten, and carries two witness signatures.
Annexure
The fixed undertaking wording on the state portal. Annexure I for a name, II for a surname, III for religion, IV for gender.
Classified text
The plain small-type newspaper booking that the gazette offices accept. A display advertisement costs many times more and buys nothing extra.
Alias
Another name you are also known by, declared alongside the new name. On the central route each extra alias carries a fee of Rs 250.
Corrigendum
A correction notice published when the original gazette entry carries an error. On the central route it is applied for within six months of publication.
Supporting proof
What the published notice is. It rewrites no register by itself. You still apply to Aadhaar, PAN, the bank and every other office separately, carrying the gazette page.
Affidavit and Notice Drafting for Kolkata, Howrah, Siliguri and Every District
The wording is the same across West Bengal. Only two steps are local: the affidavit before a Magistrate at a court in your subdivision, and the newspaper booking in a daily that circulates in your district. Send us your names and address on WhatsApp from anywhere in the state.
Gazette Name Change Notification Format: Frequently Asked Questions
What is the format of a gazette name change notification?
A short declaration in the first person: who you are (name, guardian's name, address), what changed (old name to new name), on what basis (affidavit number and where sworn), and that henceforth you shall be known by the new name and both names refer to one person. The Kolkata Gazette Part II uses this exact declaration on its portal; the Gazette of India prints the substance of your proforma in Part IV.
What does the Kolkata Gazette Part II declaration say?
In plain form: I, [applicant], son/daughter/wife of [guardian], R/o [address], have changed my Name from [existing name] to [new name] vide Affidavit No. [number] sworn before the [First Class Judicial Magistrate]. Henceforth I shall be known as [new full name] instead of [existing full name], and all my relevant documents should be corrected accordingly. It is further stated that [existing full name] and [new full name] is the same and one identical person.
What does a Gazette of India Part IV name change notice look like?
A numbered entry under Change of Name in Part IV, Section 4 of the weekly Gazette of India, carrying your old name, guardian's name, address, and the new name you shall hereafter be known by, with the date. It is printed from the proforma you signed and searchable on egazette.gov.in.
Is there a prescribed affidavit format for a gazette name change?
No single statutory form, but the content is fixed by what the gazette offices check: old name, new name, father's or husband's name, full residential address, reason, the declaration that both names are one person, and the deponent's signature in the old name. The Part II route adds that it must be sworn before a First Class Judicial Magistrate, whose name and the affidavit number are typed into the portal.
What is the newspaper notice format for a name change in West Bengal?
Three or four lines of classified text: I, [old name], son/daughter/wife of [guardian], residing at [address], have changed my name to [new name] vide affidavit dated [date] sworn before [notary or Magistrate]. Published in one Bengali and one English daily; the same wording in both.
Can I download a gazette name change sample PDF?
Yes, from the gazettes themselves. Any past Part IV issue on egazette.gov.in shows dozens of real notices; the Monthly Gazette tab on wbgazettepart2.in shows published Part II notices. Both are public records and free. This page reproduces the wording with fictional names.
Which fields must match across the affidavit, newspaper and gazette form?
Existing name, new name, guardian's name and residential address, letter for letter, including spacing, initials and spelling. The portal manual marks every field mandatory, and a mismatch is the commonest reason for a shortfall SMS or a returned central file.
Do I write the notice in the old name or the new name?
The declaration is made by the person with the old name; the affidavit, proforma and undertaking are signed in the old name. The new name is what you say you shall henceforth be known by. The new name becomes legal on publication.
What is the format for a surname change notice?
The same declaration with Surname in place of Name: have changed my Surname from [existing surname] to [new surname]. Reversion to a maiden name and deletion of a surname use the same form, with the decree or death certificate attached on the state route.
Is the gazette notification in English or Bengali?
The Part II portal declaration is in English. The 2022 guideline listed six accepted newspaper languages including Bengali; in practice we publish the newspaper notice in one Bengali and one English daily and keep the gazette declaration in English so both gazettes accept it.
Does the notification mention the reason for the change?
The affidavit states the reason. The published gazette notice usually records only the change itself, the guardian's name and address, and the declaration that both names are one person.
What is the corrigendum format if the published notice has an error?
A short application quoting the gazette issue, date and entry number, stating the error and the correction, with the original affidavit attached. Gazette of India: within six months of publication. Kolkata Gazette Part II: redressal within a week on submission of the registered document.
