CGHS Card Name Correction in Kolkata: Fix the Service Record or PPO First
A CGHS card is issued from your service record if you are serving, or your Pension Payment Order if you are retired. The card is only as right as that record. So the real fix for a wrong name is usually upstream: your department or your pension authority, then the card through the Additional Director, CGHS. For pensioners, the pension department points name changes to the 1987 DoPT rule: deed, newspaper and gazette.

CGHS Card Name Correction in Kolkata (Quick Answer)
A CGHS card copies the name in your service record or PPO. Correct that record first through your department or pension authority, then ask CGHS to amend the card; amendments are cleared by the Additional Director, CGHS. A digital card is free; a replacement plastic card costs Rs 100 on BharatKosh. A spouse added after marriage needs the marriage certificate.

| Your situation | Start with |
|---|---|
| Serving employee, name wrong on card | Your office, so the service book is right |
| Pensioner, name wrong on card | Your pension authority, so the PPO is right |
| Spouse to be added after marriage | Marriage certificate, through your office or CGHS |
| Family member's name spelt wrong | Their Aadhaar and birth or school record |
| ABHA will not link | Card details differ from Aadhaar |
CGHS in Kolkata: Who Holds a CGHS Card in West Bengal
The Central Government Health Scheme gives medical care to central government employees, pensioners and their dependent family members in cities where it runs. Kolkata is one of them, with wellness centres across the city under an Additional Director, CGHS.
Railway staff, defence staff and West Bengal state employees mostly have their own health schemes, so this page is for central government employees and pensioners who hold a CGHS card.
Each family member on the card has a beneficiary ID. A wrong name on the card can hold up a referral, a hospital admission under the scheme, or a claim.
If you are a West Bengal state government employee, your scheme is the West Bengal Health Scheme, not CGHS.
Where the Name on Your CGHS Card Comes From: Service Record or PPO
CGHS does not invent your name. A serving employee's card is made from the application forwarded by the office, which follows the service record. A pensioner's card follows the Pension Payment Order.
So if the card is wrong but your service book or PPO is right, the card simply needs amending. If the service book or PPO is wrong too, fix that first, or the new card will copy the same mistake.
The CGHS guidelines we read (OM S.11012/1/2024-EHS dated 28 June 2024) do not set out a separate name correction form. We say so plainly rather than invent one. Ask your wellness centre or the Additional Director's office for the current process.
CGHS Card Name Correction for Serving Central Employees
Ask your administrative office to check your service book. If it is right, request an amended card through the office. If it is wrong, correct it first.
For a real change of name, central government rules are the DoPT office memorandum of 12 March 1987: a deed attested by a gazetted officer, a newspaper notice, and a Gazette of India notification. See gazette notification for central government employees.
A woman taking her husband's surname after marriage only needs to intimate the office with proof. No gazette is needed for that.
CGHS Card Name Correction for Pensioners: The PPO Comes First
For pensioners, the Department of Pension and Pensioners' Welfare clarified in OM No. 3(2)/2022-P&PW(H)-7942 dated 6 October 2022 that there is no separate procedure in the pension rules for changing a name or surname in the PPO. It directs that the DoPT memorandum of 12 March 1987 be followed.
In practice that means a pensioner who has changed a name needs the deed, newspaper notice and gazette, sent to the authority that issued the PPO. Once the PPO is right, CGHS can amend the card.
A family pensioner whose name differs between the PPO and Aadhaar faces the same chain. Keep the bank account in the PPO name too.
Adding a Spouse or Correcting a Family Member on a CGHS Card
A spouse added after marriage is supported by the marriage certificate. If it has a mistake, see marriage certificate correction first.
The 2024 guidelines say the beneficiary must inform CGHS immediately of any change in the dependency of family members on the card. A daughter who marries, or a son who starts earning, may no longer be a dependant.
For a child whose name is spelt wrong, use the birth certificate or school record to correct Aadhaar, then the card.
CGHS, ABHA and Aadhaar: Why the Names Must Match
CGHS memo F.No. 44/67/MCTC/CGHS of January 2023 lets beneficiaries link their CGHS beneficiary ID with an Aadhaar based ABHA number, with Aadhaar and OTP verification. The process shows the ABHA card only when the details match.
So a card name that differs from Aadhaar can stop the link. Decide which record is right. If Aadhaar is wrong, correct it first; see name change in Aadhaar. Aadhaar allows only two name changes in a lifetime.
Getting Your Amended CGHS Card: Digital Free, Plastic Rs 100 on BharatKosh
Under the 2024 guidelines, the digital card is free, and a plastic card replacement costs Rs 100, paid through BharatKosh.
Once the amendment is cleared by the Additional Director, you can download the revised card. Check every family member's name and date of birth on it before you use it at a hospital.
Keep the old card until the new one is issued, and take an acknowledgement for anything you submit.
When a CGHS Card Name Change Needs a Gazette
A spelling mistake on the card does not need a gazette. Nor does a married surname. We say so even though gazettes are our work.
A gazette is needed when a serving employee or pensioner has really changed a name, because the 1987 DoPT rule asks for one. For central staff and pensioners, the Gazette of India route fits, in seven steps: make a name change affidavit before a notary; publish a newspaper notice and keep the original page; fill the typed proforma with two witnesses; you and both witnesses sign in the old name; pay on BharatKosh Rs 1,100 for an adult (revised each financial year, non-refundable); post the file in your own name to the Department of Publication, Civil Lines, Delhi 110054, since agents cannot submit it; and after 30 to 45 days download it free from egazette.gov.in.
The gazette is supporting proof. It changes nothing by itself; the service book, PPO and CGHS card are each amended in turn.
