West Bengal Health Scheme Card Name Correction: Enrolment Certificate, Aadhaar and Service Book
Under the West Bengal Health Scheme, your "card" is the digitally signed Enrolment Certificate issued by your Head of Office or, if you are retired, your Pension Sanctioning Authority. A Finance Department memo of 2 June 2026 asked every employee and pensioner to give Aadhaar numbers and check the certificate's photos, signatures and blood group by 31 July 2026. That check is the right moment to look at every name on it. This page shows who corrects it and in what order.

WBHS Card Name Correction (Quick Answer)
A wrong name on a West Bengal Health Scheme Enrolment Certificate is corrected by the office that issues it: your Head of Office if you are serving, or your Pension Sanctioning Authority if you are retired, on the WBHS portal. Give them Aadhaar and proof of the correct name. A spelling slip needs no gazette; a real change of name by a state employee needs a deed, a newspaper notice and the Kolkata Gazette (Rs 1,100).

| Problem | Who to approach |
|---|---|
| Your name spelt wrong on the certificate | Head of Office or Pension Sanctioning Authority |
| Family member's name spelt wrong | Same office, with that person's Aadhaar |
| Wife's maiden name on certificate | Same office, with the marriage certificate |
| You really changed your name | Service book first, then the certificate |
| Aadhaar itself is wrong | UIDAI first, then the office |
What the West Bengal Health Scheme (WBHS) Card Really Is: The Enrolment Certificate
The West Bengal Health Scheme covers West Bengal state government employees, pensioners, All India Services officers of the state and some grant-in-aid institution staff, with cashless treatment at listed hospitals and reimbursement of claims.
People call it the WBHS card, but the document is an Enrolment Certificate from the WBHS portal. It lists every enrolled family member with a Beneficiary ID, photograph, signature and blood group, and it is digitally signed by the office that approved it.
Because the certificate is digitally signed by your office, you cannot edit it yourself. Any change goes back to that office.
Central government employees in Kolkata have CGHS instead; see CGHS card name correction.
The June 2026 WBHS Memo: Aadhaar and a 31 July Deadline
Finance Department (Medical Cell) memo No. 51-F(MED)WB dated 2 June 2026 set 31 July 2026 as the last date to complete enrolment for government employees, pensioners and All India Services officers.
It asks beneficiaries to give their Aadhaar numbers to their office if they have not already, and to check on the WBHS portal that their digitally signed certificate carries photographs, signatures and blood group. Heads of Office and Pension Sanctioning Authorities must issue complete certificates and help add Aadhaar.
After 31 July 2026, the memo says claim settlement needs approval from the Finance Department's Medical Cell. If you missed the date, ask your office what that means for you.
Who Corrects a Name on a WBHS Enrolment Certificate: Head of Office or Pension Authority
For a serving employee, the Head of Office (through the office's WBHS login) approves and signs the certificate, so ask that office to correct a spelling.
For a pensioner, the Pension Sanctioning Authority issues the pensioner's certificate. The memo says an employee retiring within a year continues in the scheme unless they opt out, with the PSA issuing the pensioner certificate at superannuation. Check the spelling at that point.
Give a written request with a copy of Aadhaar and one more ID, and keep a received copy. The WBHS guidelines we read do not set out a separate name correction form, so we do not invent one.
Family Member Names on the WBHS Enrolment Certificate
Family under WBHS can include a spouse, dependent parents, dependent children up to 25, and some dependent daughters, sisters and minor siblings, subject to income limits in the 2021 grant-in-aid notification and the scheme rules.
Each member's name now sits next to an Aadhaar number. A mother entered as "Smt. Das" or a child entered by a pet name will not match. Correct the entry through your office with that member's Aadhaar and birth or school record.
If Aadhaar itself is wrong, correct it first; see name change in Aadhaar.
Wife's Maiden Name and Married Name in WBHS
A wife added under her maiden name, whose Aadhaar now shows her married name, is an example of two true names. The proof that links them is the marriage certificate.
For a woman employee changing her own surname after marriage, West Bengal's Finance Department memo 3990-F of 4 May 2005 treats it as an intimation, not a gazette case. Tell your office with the marriage certificate, and ask it to update both the service book and the WBHS certificate.
If the marriage certificate has a mistake, see marriage certificate correction first.
A Real Change of Name by a State Employee or Pensioner: Memo 3990-F of 2005
When a state employee really changes a name, not just a spelling, memo 3990-F of 4 May 2005 asks for a deed changing the name attested by a gazetted officer, a newspaper notice and publication in the Kolkata Gazette, at the employee's own cost. The service book entry is what changes the record.
Once the service book carries the new name, the Head of Office can correct the WBHS certificate. See name change for West Bengal state government employees.
Pensioners follow the same idea through the Pension Sanctioning Authority. A gazette is supporting proof; it does not change the certificate by itself.
Kolkata Gazette Part II for a WBHS Name Change
State employees use the state gazette, the Kolkata Gazette Part II, which also has a category for government employees.
Swear an affidavit before a First Class Judicial Magistrate; a notary will not do. Publish a notice in one Bengali and one English newspaper and keep the full pages. Apply at wbgazettepart2.in with ID (up to 2 MB) and photo (up to 100 KB), pay Rs 1,100 on GRIPS, and the notification usually comes in about 7 days with an SMS. See Kolkata Gazette Part II online application.
Gazette of India Route, If You Are Asked for It
Some beneficiaries, such as All India Services officers or family members living outside West Bengal, may be asked for the Gazette of India instead.
The seven steps: make a name change affidavit before a notary; publish a newspaper notice and keep the original page; fill the typed proforma with two witnesses; you and both witnesses sign in the old name; pay on BharatKosh Rs 1,100 for an adult, Rs 1,700 for a minor or Rs 3,500 from abroad (revised each financial year, non-refundable); post the file in your own name to the Department of Publication, Civil Lines, Delhi 110054, since agents cannot submit it; and after 30 to 45 days download it free from egazette.gov.in.
There is no central gazette office in West Bengal.
Mistakes With WBHS Name Corrections
Trying to edit the certificate yourself
Only your office can.
Correcting WBHS but not the service book
The next certificate repeats the error.
Ignoring the 31 July 2026 date
Later claims need Medical Cell approval.
A gazette for a married surname
Intimation with the marriage certificate is enough.
Leaving a family member's pet name
It will not match Aadhaar.
No copy of your request
Keep a received copy.

Ask the office that issues your Enrolment Certificate: your Head of Office if serving, or your Pension Sanctioning Authority if retired, with Aadhaar and proof of the correct name.
Can I edit my WBHS certificate on the portal myself?
No. It is digitally signed by your office, so changes go through that office.
What did the June 2026 WBHS memo say?
Memo 51-F(MED)WB of 2 June 2026 set 31 July 2026 to complete enrolment, asked for Aadhaar numbers, and required complete certificates with photos, signatures and blood group.

Memo 3990-F of 4 May 2005: a deed attested by a gazetted officer, a newspaper notice and the Kolkata Gazette, then the service book entry.
