Swasthya Sathi Card Name Correction in West Bengal: Ask for Form C and Turn Your Card Over First
The scheme uses four different forms and a name correction is Form C, so asking for the wrong one wastes the trip. The state bears the entire premium including the cost of the smart card, which means a correction is not something to pay an agent for. And the toll free helpline is printed on the back of your own card, which is the quickest place to start.

Swasthya Sathi Name Correction (Quick Answer)
A name correction is Form C. The scheme keeps separate forms for separate jobs: Form B enrolment, Form A addition, Form C correction, Form D deletion. Before travelling, check what is actually recorded using Find Your Name on the official data portal, which is free. Then turn your card over and call the 24 hour toll free number 1800 345 5384, which the official portal says is displayed on the back of the smart card. Corrections go to your District Swasthya Sathi office, where the District Key Manager is the Additional District Magistrate (Health). The scheme costs the beneficiary nothing, since the state bears the premium and the cost of the smart cards. The one exception: a duplicate card for a lost card is at the beneficiary's cost. No gazette is needed for a correction.
| Your situation | What you need | Cost |
|---|---|---|
| A recorded name is spelt wrongly | Form C, name correction | Free |
| A family member is missing from the card | Form A, name addition | Free |
| A name should be removed | Form D, name deletion | Free |
| Family not enrolled at all | Form B, enrolment | Free |
| Card lost, need a duplicate | District office | Beneficiary bears it |
| Card names the eldest woman, not you | Nothing. That is by design | Nil |

Swasthya Sathi Forms A, B, C and D: Form C Corrects Your Name
Ask for the right one by name. Being handed the addition form when you need the correction form is the commonest wasted journey on this subject.
Form B: Enrolment
Registration under the scheme, for a family not yet enrolled.
Form A: Name addition
To add a family member to an existing card.
Form C: Name correction
The one you want if a recorded name is wrong.
Form D: Name deletion
To remove a name from the card.
Form versions change, so ask your district office to confirm the current form before you fill anything in. What matters is that you ask for the correction form and not the addition form, because they do different things and one will not do the other's job.
Swasthya Sathi Name Correction Fee: What It Costs You
The official portal is explicit. There is no beneficiary contribution: nothing is to be paid towards the insurance premium or the card cost, because the entire premium, including the cost of the smart cards, is borne by the Government of West Bengal.
So a name correction is not something anybody should be charging you a fee to arrange. Including us. This page is here because the honest answer is that you can do it yourself.
There is one exception, and it is fair to know it. District information states that where a card is lost and a duplicate is needed, the cost of making the duplicate has to be borne by the beneficiary. A correction and a replacement are not the same thing, so be clear which you are asking for.
Look after the card
The card is free to receive and valid for a lifetime, auto renewed each year. A replacement is not free. That is a good reason to keep it somewhere safe rather than in a bag.
Check Your Swasthya Sathi Record Online Before You Travel
The official Swasthya Sathi data portal has a Find Your Name facility. Use it before you go anywhere.
It shows you the names actually recorded for the family, which is often different from what people assume from looking at the printed card. Sometimes the record is right and only the reading of it was wrong. Sometimes a name people thought was missing is there.
Ten minutes on that page can save a journey to a district office, and it is free.
Use only the official portal, whose address ends in gov.in. Do not enter Aadhaar or card details on any site that does not.
Two other official facilities
The portal also carries new enrolment data entry and an application status check. If somebody has applied on your behalf and you have heard nothing, the status page is worth a look.
Swasthya Sathi Helpline 1800 345 5384: The Number on Your Card
This is our favourite detail about this scheme, because it means the first step costs nothing and requires no website.
There is a 24 hour toll free call centre on 1800 345 5384, and the official portal notes that the number is displayed on the back of the smart card itself.
So the fastest way to begin is to turn your own card over and ring the number printed there. Ask what your district requires for a name correction and which form to bring.
There is also an official Swasthya Sathi mobile app on the Play Store, which can raise queries and grievances and has a one touch feature that connects directly to the helpline.
And an online grievance route
The portal carries an online grievance monitoring mechanism. Logging a grievance gives you a record, which is worth having even if you also go in person.
Why the Swasthya Sathi Card Is in the Eldest Woman's Name
A common worry, and usually a false alarm.
The official portal states that the card is issued preferably in the name of the eldest woman family member as head of family, and describes this as a deliberate feature rather than an administrative accident.
So if your card carries your mother's or mother-in-law's name as head of family, nothing has gone wrong. What you should check instead is that your own name appears among the family members recorded on the card, and that it is spelt correctly.
The scheme also places no cap on family size and covers parents of both spouses, so a large household is not a reason for anybody to be left off.
Check the member list, not the header
The name at the top is the head of family. The list that matters to you is the members. Find Your Name shows both.
Swasthya Sathi District Office for Name Correction
Corrections and additions go to the District Swasthya Sathi office, or to a district kiosk where your district runs one.
District administration information describes the District Key Manager for the scheme as the Additional District Magistrate (Health), with the Deputy Chief Medical Officer of Health also involved in running it. Those are the designations to ask for if you are being passed around.
Arrangements differ between districts, and some run kiosks at subdivisional offices as well as at the district headquarters. Ask on the helpline where your nearest point is before travelling, particularly if you are in a large or hilly district.
Ask for a receipt
Whatever you submit, ask for an acknowledgement. It is how you follow up, and it turns a conversation into a record.
Why a Swasthya Sathi Correction Is Not Just a Card Reprint
People sometimes expect a corrected card the same afternoon, and it helps to understand why that is not how it works.
Swasthya Sathi runs on a smart card that carries digitally encrypted information alongside the printed details: family members, photographs, biometrics, address and mobile number. The whole system is paperless and IT based.
So a correction means the underlying record is put right and then reflected on the card. That is more than a reprint, which is why it takes some days rather than minutes.
Ask the office how long your district is currently taking and what you need to come back with. Getting both answers at the first visit is the whole trick.
Meanwhile
If you need treatment before the correction is done, say so at the office. Districts have issued cards urgently in genuine cases, so it is worth explaining the situation rather than waiting silently.
When a Swasthya Sathi Name Error Holds Up Other Schemes
Swasthya Sathi enrolment is used as a condition in other West Bengal schemes, so a wrong or missing name on this card can quietly hold up something you applied for elsewhere.
If more than one thing has stopped working, say so at the office rather than treating them as unrelated problems. A single wrong entry can be the common cause, and fixing it once can release both.
Where a welfare payment fails over a name or Aadhaar mismatch, which has its own free fix at your bank.
Aadhaar first, as always
If your Aadhaar itself carries the wrong name, correct that first, because most records are checked against it. The Aadhaar route.
No Gazette Needed for Swasthya Sathi Name Correction
If a name has been recorded wrongly on the card, that is a data correction. It goes on Form C to the district office, supported by identity proof carrying the right spelling. A gazette notification would add nothing.
A gazette becomes relevant only where your own legal name has genuinely changed, and even then the card correction still goes through the district office with the gazette as supporting proof.
So if somebody offers you a gazette to fix a misspelt name on a health card, that is the wrong document for the job.
When a spelling needs no gazette at all, and when a gazette genuinely is needed.
What we can help with
Very little here, honestly. Where your legal name has changed we prepare the gazette and the supporting papers. For a plain card correction, the district office and the helpline are free and are the right route. What a consultant can and cannot do.
Swasthya Sathi Name Correction: Frequently Asked Questions
Which form do I need for a name correction?
Form C is the name correction form. The scheme uses separate forms for different jobs: Form B for enrolment, Form A to add a name, Form C to correct one, and Form D to delete one. Ask your district office to confirm the current version before filling anything, and ask for Form C by name so you are not handed the addition form instead.
Does it cost anything?
The scheme itself costs the beneficiary nothing. The official portal states there is no contribution towards the insurance premium or the card cost, because the entire premium including the cost of the smart cards is borne by the Government of West Bengal. So a correction is not something you should be paying an agent for.
Is there anything I do have to pay for?
Yes, one thing, and it is worth knowing. If the card is lost and you need a duplicate, district information states the cost of making the duplicate card has to be borne by the beneficiary. So look after the card. A correction is not the same as a replacement.
The card is in my mother-in-law's name. Is that wrong?
Almost certainly not. The official portal says the card is issued preferably in the name of the eldest woman family member as head of family. That is deliberate, and it is described as gender empowerment rather than an error. Your own name should appear among the family members on the card, which is the thing to check.
How do I check what name is actually recorded?
Use the Find Your Name facility on the official Swasthya Sathi data portal before you travel anywhere. It is free and immediate, and it shows the names recorded for the family. Doing that first tells you whether you have a correction to make at all.
Where do I go for the correction?
The District Swasthya Sathi office, or a district kiosk where your district runs one. District information describes the District Key Manager as the Additional District Magistrate (Health), with the Deputy Chief Medical Officer of Health also involved, so those are the offices to ask for.
Is there a helpline?
Yes, a 24 hour toll free call centre on 1800 345 5384, and the official portal notes that the number is displayed on the back of the smart card itself. So the fastest way to start is to turn your own card over. There is also a Swasthya Sathi mobile app on the Play Store with a one touch feature that connects to the helpline.
Can I raise it online instead of travelling?
There is an online grievance mechanism on the official portal, and the mobile app can raise queries and grievances too. For a data correction you will usually still need to deal with the district office, but logging the grievance first gives you a record and sometimes gets you an answer without a journey.
Why does a correction take a few days?
Because this is a smart card scheme. The card carries digitally encrypted information as well as printed details, so a correction is not simply a reprint; the record has to be corrected and then reflected on the card. Ask the office how long your district is taking and what you should come back with.
Do I need a gazette notification?
No, not for a correction. If a name is recorded wrongly, that is a data correction on Form C. A gazette would only come into it if your own legal name has genuinely changed, and even then the correction to the card still goes through the district office.
Does a name error here affect anything else?
It can. Swasthya Sathi enrolment is used as a condition in other West Bengal schemes, so a wrong or missing name here can quietly hold up something else you have applied for. If more than one thing has stopped working, mention that at the office rather than treating them as separate problems.
What should I take with me?
Your smart card, your Aadhaar, and identity proof showing the correct spelling. If a family member's name is the problem, take their documents too. Ask the office for its own list first, because a wasted trip costs more than a phone call.
