EPS Pension Form 10D Name Mismatch in West Bengal
Form 10D is how a member, a widow or widower, children, a nominee or a dependent parent claims a monthly pension under the Employees' Pension Scheme. Who Uses EPS Form 10D: Member, Widow, Child, Guardian, Nominee or Dependent Parent
EPFO's Form 10D instructions list who can use it: the member, a widow or widower, a major child or orphan, a guardian, a nominee, and a dependent parent.
It asks for the descriptive roll and specimen signature or thumb impression in duplicate, three passport size photographs, the employer's certificate of wage particulars, age proof for children from the school, the birth registrar or ESI records, a guardianship certificate if the guardian is not the natural guardian, and a death certificate where relevant. Names are written in block letters.
Many members now file the pension claim online through the member portal once UAN, Aadhaar and bank details are linked and verified. The paper form is still used, especially for family claims.
Either way, the name test is the same: the person's name must agree across records.

EPS Widow or Widower Pension When the Name Differs From Family Details
For a widow's or widower's pension, EPFO compares the claimant with the family details the member gave. A wife recorded by her maiden name, a short name or a pet name, while her Aadhaar shows another, needs a link.
Carry the member's death certificate, the claimant's Aadhaar and bank passbook, the marriage certificate, and an affidavit that the names are of one person; see one and the same person affidavit.
If the marriage certificate has a mistake, correct it first; see marriage certificate correction. A gazette is not normally needed for this.
EPS Children and Orphan Pension: Age Proof and Names
For children's pension, the instructions ask for age proof from the school, the Registrar of Births and Deaths, or ESI records. The child's name on that proof should match the name in the claim and on Aadhaar.
If a child's birth certificate carries no name, or a different one, correct it at the local body that registered the birth; see name change in birth certificate.
An orphan's claim through a guardian who is not the natural guardian needs a guardianship certificate from the court.
Bank Account for EPS Pension: Same-Branch Rule and CPPS From January 2025
The older Form 10D instructions say the member, spouse and children should open savings accounts in the same branch of the bank.
Since January 2025, EPFO's Centralised Pension Payments System lets pensioners receive pension from any bank, anywhere in the country, and was reported as live in all 122 regional offices for over 68 lakh pensioners. The two do not fully match, so ask your EPFO office what it wants for a new claim.
Whatever the bank, the account must be in the claimant's own name, spelt the same as the claim.
Form 10D When the Employer Has Closed: Who Attests It
The instructions say the application goes through the establishment where the member last served or died. If that employer has closed, it can be attested by a magistrate or gazetted officer instead.
Take the attesting officer the claimant's ID and the name proof, so the attestation and the claim show the same name.
