Its instructions say it is submitted at the APY-SP bank branch, that subscribers must give relevant documentary proof as asked by the bank, and that an application incomplete or without the required documents is liable to be rejected.
Fill it in block letters, and keep the stamped acknowledgement. Your PRAN and contributions do not change.
Which Proof to Carry for an APY Name Correction: Aadhaar and Passbook
The APY form does not list exact documents; it leaves that to the bank. In practice, carry your Aadhaar, since APY uses Aadhaar for KYC, and your bank passbook.
