ASHA Worker Selection Name Mismatch in West Bengal
Every ASHA (Accredited Social Health Activist) applicant in West Bengal is, by the rules, a married, divorced or widowed woman who lives in the village or ward she will serve. That one rule explains almost every name problem in ASHA selection. Her Madhyamik admit card and marksheet are in her maiden name, issued from her parents' home; her voter card, ration card and Aadhaar now carry her husband's surname and his village address. The selection committee has to see that these papers belong to one woman. This page explains what ASHA notices ask for, why the marriage registration certificate matters so much, what to do if the marriage was never registered, and how widows and divorced women prove their status.

ASHA Selection Name Mismatch in West Bengal (Quick Answer)
ASHA applicants must be married, divorced or widowed, so a maiden name on Madhyamik and a married name on voter card, ration card and Aadhaar is normal. Link them with the marriage registration certificate, which ASHA notices ask for, plus an affidavit if spellings also differ. Widows attach the husband's death certificate; divorced women the decree. Fill the form in your Madhyamik name, since Madhyamik marks decide the merit.
| Problem | Fix |
|---|---|
| Maiden name on Madhyamik, married name elsewhere | Marriage registration certificate and affidavit |
| Marriage never registered | Register it before the notice closes |
| Husband's death certificate has a wrong name | Correct it with the Registrar, or affidavit |
| Divorced, using maiden name again | Divorce decree and affidavit |
| Voter card still at parents' address | Shift voter registration to the service area |
What West Bengal ASHA Selection Notices Say
ASHA posts are filled sub-centre by sub-centre through notices issued by Sub-Divisional Officers or district authorities. A Barrackpore Sub-Division notice (Memo No. 436/Con/Bkp, dated 28 March 2023) and a Malda Sadar notice for 2026 set out the same core rules.
The applicant must be a married, divorced or widowed woman, aged 30 to 40 (with the lower limit relaxed to 22 for SC and ST candidates), who has passed or appeared in Madhyamik or equivalent, and who is a permanent resident of the service area she applies for.
Selection is on Madhyamik marks and an interview. The Malda 2026 notice gave 90 per cent weight to Madhyamik marks and 10 per cent to the interview, with applications online through the district website.
Documents Asked in ASHA Applications in West Bengal
The Barrackpore notice asked for self-attested copies of: birth certificate or Madhyamik admit card; voter card, Aadhaar and ration card; Madhyamik marksheet; marriage registration certificate, divorce decree or the husband's death certificate; SC or ST certificate if claimed; and SHG or trained dai certificates if any.
Look at that list again. It mixes papers issued before marriage (admit card, marksheet) with papers issued after (voter card, ration card at the husband's address). The marriage, divorce or death document is the bridge between them.
The selection committee will compare every name and every father's or husband's name across these papers.
Which Name to Use in an ASHA Application
Use your name exactly as on your Madhyamik admit card and marksheet, because Madhyamik is your qualification and its marks decide your rank. Then show how that name became your present name.
For a woman whose name has changed only by taking her husband's surname, the marriage registration certificate usually does it. If the first name or spelling also differs (Rina on Madhyamik, Reena on Aadhaar), add an affidavit listing every version. See affidavit for name change after marriage.
If the online form asks for the name as on Aadhaar, enter it as asked, and keep the affidavit ready for document verification.
Marriage Registration Certificate: The Key ASHA Document
ASHA notices ask specifically for a marriage registration certificate. Many marriages in rural West Bengal were never registered with the Marriage Registrar. If yours was not, register it now; registration after the wedding is allowed, with witnesses and proof.
If the marriage certificate itself has a wrong name or spelling, correct it before you apply. See marriage certificate correction.
Do not leave registration until the notice is out. ASHA application windows are often only a week or two long.
Widows and Divorced Women Applying for ASHA Posts
A widow attaches her husband's death certificate. If his name in that certificate differs from his name on her voter card or ration card, the committee may ask questions. Correct it with the Registrar who issued it, or add an affidavit. See death certificate name correction.
A divorced woman attaches the divorce decree. If she has gone back to her maiden name, her Madhyamik name and present name may already match; if not, see name change after divorce.
Widow pension papers often show the same names; if they differ too, see widow pension name mismatch.
Residence Proof in the Service Area After Marriage
ASHA posts are tied to a sub-centre area, and the applicant must live there. After marriage, your voter card and ration card should show your husband's address in that area, in your married name.
If your voter card is still at your parents' address, shift it; see voter ID after marriage. For the ration card, see adding a wife's name to the ration card.
Some notices accept other residence proof; read yours closely and ask the block office if unsure.
Madhyamik Marks Decide ASHA Merit, So the Name Must Match
Because ranking rests mainly on Madhyamik marks, the committee must be sure that the marksheet belongs to you. That is why the link from your maiden name to your present name matters more here than in many other jobs.
If your Madhyamik certificate has a spelling error, correct it with the board through your school if time allows; otherwise link it by affidavit. See Madhyamik marksheet name correction.
Anganwadi worker and helper selection works in a similar way; see Anganwadi recruitment name mismatch.
