Name Change in Pension and Family Pension Records in West Bengal
A government pension rests on one document: the Pension Payment Order (PPO). It names the pensioner and, usually, the spouse who will get family pension later. When the pensioner's name in the PPO differs from Aadhaar or the bank, the monthly pension can stall at the treasury. When the spouse's name in the PPO differs from her own papers, family pension is held at the worst possible time, just after a death. West Bengal state pensioners get an e-PPO from the Directorate of Pension, Provident Fund and Group Insurance (DPPG), sent to the treasury through IFMS. Central pensioners' PPOs come from their department's pension authority, and DoPPW's 6 October 2022 OM says a PPO name change follows the DoPT procedure of 1987. Fix the PPO through the office that issued it, then the bank and Aadhaar. For family pension, the spouse's name in the PPO must match her papers; carry an affidavit if it does not.

Who Issues the Pension Payment Order in West Bengal
For West Bengal state government pensioners, the Directorate of Pension, Provident Fund and Group Insurance (DPPG) issues the PPO. Under G.O. No. 2546-F(Y) dated 21 April 2017, PPOs are issued as e-PPOs and sent to treasuries through the state's IFMS system. The treasury officer verifies the pensioner's identity under the West Bengal Treasury Rules, 2005.

Name Correction in a West Bengal State Pension PPO
A West Bengal state pensioner's PPO is built from the service book. If the PPO name is wrong because the service book was wrong, the correction starts with the last office you served in, which corrects or confirms the service record and approaches DPPG.

Write to your former Head of Office with the PPO copy, ID in the correct name and the service book entry, and ask for the correction to be forwarded to DPPG. Keep the receiving copy.
Name Change in a Central Government PPO
The Department of Pension and Pensioners' Welfare clarified in OM No. 3(2)/2022-P&PW(H)-7942 dated 6 October 2022 that pension rules have no separate procedure for changing a name in a PPO, and that the DoPT memorandum of 12 March 1987 should be followed.
In practice, a central pensioner who has changed a name provides the deed, newspaper notice and Gazette of India notification to the PPO-issuing authority. See gazette for central government employees.

Spouse's Name in the PPO for Family Pension
The PPO usually names the spouse who will receive family pension. If her name there is spelt differently from her Aadhaar, bank account or the marriage certificate, family pension can be delayed after the pensioner dies.
Check the spouse's name in the PPO now, while the pensioner is alive, and ask the issuing office to correct it. It is far harder to do later, when only the widow is left to prove who she is.

Family Pension After the Pensioner's Death: Name Mismatches
To start family pension, the spouse usually gives the death certificate, her ID and bank details to the treasury or pension office. If the death certificate spells the pensioner's name differently from the PPO, questions follow; see death certificate name correction.
If the spouse's own name differs between the PPO and her ID, attach an affidavit stating that both names belong to her. See one and the same person affidavit.
Ask the office in writing what it needs, and keep copies of everything you give.
Age Proof for Family Pensioners at 80 in West Bengal
West Bengal family pensioners get additional pension from age 80. A circular of the Accountant General (A&E), West Bengal lists the documents accepted as age proof, including the Madhyamik certificate, a medical certificate, voter card, PAN and, added on 22 May 2023, Aadhaar.
The same circular says the date of birth first recorded and accepted for family pension is final and cannot be changed later on the basis of other documents.
So check the date of birth and name recorded at the start of family pension carefully. Errors there stay.
Bank Account and Life Certificate Names for Pensioners
Pension is credited to the bank account named in the PPO. If the bank's name for you differs, credits can fail. Update the bank with the PPO copy and Aadhaar.
