PCC for a Job or Visa Abroad: Use Your Passport Name
A PCC for work or immigration abroad is read by a foreign employer, an embassy or an immigration officer. All of them have your passport in front of them. If the PCC says "Md. Rafiq Sk" and the passport says "Mohammad Rafique Sheikh", they may reject it.
So apply in the name exactly as on the passport, letter for letter. The Home Department's note lists passport details among the documents for a PCC, which is why the passport decides.

PCC Through Passport Seva Versus the State Police
Many overseas employers accept a PCC issued through the Passport Seva system of the Ministry of External Affairs, which is issued against your passport and so carries the passport name automatically. Others ask for a state police PCC. Ask the employer or embassy which one they want before you apply.
Because the Passport Seva PCC follows the passport, a name mismatch there is really a passport question. Because the state police PCC is issued on your application, a mismatch there usually comes from writing a different form of your name on the form.
Either way, the cure is the same: make the passport right, and apply in that name.
Character Certificate for a Job in India
For a job or admission in India, the reader compares the certificate with your application form, your Aadhaar and your school certificates. Apply in the name you used on the job application, which should be the Aadhaar name.
If you are a married woman and your Aadhaar is in the married name but your certificates are in the maiden name, put both names on the application where the form allows, and keep a marriage certificate or affidavit ready.
A character certificate is a statement about your conduct. It is not proof of your name. Do not use it to try to fix a name gap; fix the name in the records.

For a government job the employer sends your attestation form to the police for verification. The police check the name, father's name, address and antecedents on that form against local records.
