How to Correct Your Name in CMPF Records Through the Colliery Personnel Office
Go to the personnel department or CMPF section of your colliery, unit or area office. Give a written request with Aadhaar, PAN if you have it, your bank passbook and the proof that fits: your Class 10 certificate for a spelling error, the marriage certificate for a married surname, or an affidavit linking two forms of your name; see one and the same person affidavit.
Ask the office to correct the service record first, then send the corrected particulars to CMPFO. Keep a dated acknowledgement.

CMPF Form A Nomination: Nominee and Spouse Names That Must Match
CMPFO's procedure says that if a valid Form A free from tampering is available, the claim is taken from the nominee only. So the nominee's name on Form A decides who gets paid.
Write each nominee's name as their Aadhaar shows it. A wife entered by a pet name, or a nominee who has since died, makes a future claim harder. Update Form A through your unit.
Without a valid nomination, the claim goes to eligible survivors under the CMPF and pension schemes, and the family must prove who they are.
Fix Your CMPF Name Before Retirement: The Six-Month SAHAJ Window
The procedure asks personnel departments to issue the superannuation notice six months before retirement, with the SAHAJ claim form. That six month window is your last easy chance to correct a name.
Claims need a copy of the savings bank passbook, preferably the salary account, a cancelled cheque, Aadhaar and the employee ID. If your bank account or Aadhaar shows a different name from CMPF, correct them before the claim goes in.
The member's signature or thumb impression is taken in front of the CMPF staff, so sign the same way you sign your service papers.
CMPF Death Claims When a Name Does Not Match the Death Certificate
For a death claim, the family gives the death certificate from the Registrar of Births and Deaths, and guardianship or legal heir proof where needed. The procedure expects death claims to be processed within two weeks of receipt.
If the widow's name in the company record differs from her Aadhaar, or the member's name differs between records, the unit forms a fact finding committee of three executives to resolve it. That takes time.

CMPFO's C-CARES portal, launched on 31 January 2024, lets subscribers view their details and subscription status. Version 2.0, launched on 3 June 2025, added a mobile app to view the profile, track claims, raise grievances and see PF balances, starting with regional offices including Asansol-I.
Use it to check your name, but make corrections through your unit, since the company submits the data.
