Adding a Child's Name to a Ration Card in West Bengal
A new baby, or a child who was left off the family card, is added to an existing West Bengal digital ration card with Form 4, the form for including a left out family member. A child under five does not need Aadhaar for this; the birth certificate is enough. The name the card prints comes from the document you attach, so the birth certificate has to carry the child's correct name before you apply. This page shows the right form, the right papers and the right order.

How to Add a Child to a West Bengal Ration Card (Quick Answer)
Use Form 4, inclusion of a left out member in an existing digital ration card. Attach a copy of the family card, address proof and the child's Aadhaar. If the child is below five and has no Aadhaar, the department accepts the birth certificate instead. Apply online, at a Bangla Sahayata Kendra or at your food office. There is no government fee.
| Your situation | Form to use |
|---|---|
| Newborn or child not on any card, family already has a WB digital card | Form 4, inclusion of a left out member |

| Child is on grandparents' or another family's card | Form 14, shift an individual to another family |
| Child is on the card but the name or age is wrong | Form 5, rectification of details |
| Family moved to a new area | Form 6, change of ration shop |
Form 4: Adding a Child to an Existing Ration Card in West Bengal
The Food and Supplies Department's common application form for digital ration cards gives each change its own form number. Adding a child to a family that already holds a West Bengal digital ration card is Form 4, which the department describes as inclusion of left out family members in an already existing family.
The same form is used whether the child is a newborn, a toddler who was never added, or an older child whose name was missed when the family first applied. The card number stays the same; the child is added as a new member under it.
Form 4 works for cards under NFSA, the Rajya Khadya Suraksha Yojana (RKSY) and the general category. The department's urban common form lists Form 4 as covering all three.
If you are not sure which category your card is, the printed e-ration card shows it. You do not choose the category for the child; the child joins the family's card as it stands.
Older Guides Say Form 3 and Age Above Five: What Changed
Some older guides on adding a child to a West Bengal ration card tell parents to fill Form 3, attach the original ration card and the birth certificate, and say the child must be above five years old. That advice comes from the paper card system.
On the current digital card forms, Form 3 is the application for a whole new family under NFSA or RKSY, and Form 10 is the same for the general category. Form 4 is the one for adding a member to a card that already exists. The current form also makes a direct provision for children below five, so there is no rule that the child must first turn five.
Where an older guide and the department's current form disagree, we follow the department's form. If your office tells you something different, go by what the office accepts, because it is the office that processes the form.
Documents Needed to Add a Child's Name to the Ration Card
For Form 4 the department's common form lists a copy of the existing digital ration card, the Aadhaar of the new member, and address proof with a clear, readable address.
For a child below five who does not have Aadhaar, the form says Aadhaar is not mandatory and a copy of the child's birth certificate is submitted instead. A child of five or more is expected to have Aadhaar.
- Copy of the family's digital ration card
- Child's birth certificate (under five, without Aadhaar)
- Child's Aadhaar (five or older, or if already made)
- Address proof with a readable address
- Aadhaar of the parent or head of family filling the form
The Child's Name on the Birth Certificate Decides the Ration Card Name
The ration card does not invent a name. It prints the name from the document you attach, which for a young child is the birth certificate. If that certificate is wrong, the ration card will be wrong too, and every scheme that reads the card will carry the mistake.
A common West Bengal problem is a birth registered at the hospital before the parents had picked a name, so the certificate has no name at all. That is not a ration card matter. The name is added by the Registrar of Births and Deaths of the body that registered the birth, the Kolkata Municipal Corporation in Kolkata or your municipality or gram panchayat elsewhere. See name change in birth certificate for a child for how that works.
Fix the certificate first, then apply for the ration card. It takes a little longer at the start but saves a second correction later.
When the Child Gets Aadhaar, the Names Must Match
A child added with only a birth certificate will later get Aadhaar, often Baal Aadhaar before five. Ration card details are linked to Aadhaar, so the spelling on the card and on the child's Aadhaar should be the same.
If Aadhaar is made with a different spelling, decide which one is right. If Aadhaar is wrong, correct it at a Seva Kendra; see child Aadhaar name change. If the ration card is wrong, Form 5 rectifies the name, age or guardian name on the card.
Keeping the two the same matters later, because schemes such as Kanyashree and school scholarships check one record against another.
Child Already on Grandparents' or Another Family's Card
Sometimes a child was added to the grandparents' card, or stayed on the mother's parents' card after a move. Adding the child to your card with Form 4 would put one child on two cards.
The department lists Form 14 for shifting an individual from one family's card to another existing family's card. Its checklist asks for both cards and proof of the relationship. One child should be on one card only.
The same rule applies to a wife joining her husband's card after marriage; see adding a wife to the ration card.
Where to Apply: Online, Bangla Sahayata Kendra or Food Office
You can apply through the Food and Supplies Department's portal at food.wb.gov.in, at a Bangla Sahayata Kendra, or at the food office for your area: the Inspector of Food and Supplies in rural areas, the rationing office in Kolkata and other urban areas. The forms are free.
An older guide says the application is checked on the ground by a ward or gram panchayat official before the card is updated, and gives 2 to 3 weeks after that check. Treat that as a rough guide, not a promise.
The same guide gives the department's toll free numbers as 1800 345 5505 and 1967. Do not pay anyone to fill in a free form.
